Articulation Agreement Instructions
- Download the articulation agreement .
- Print the application and help student fill in completely, including signatures of student, instructor, and counselor.
- Turn in application to Instructor.
- Counselor will sign application, and attach the student transcript, and send to:

Once the agreement is approved, your guidance counselor will be notified and you will be assigned a user id and password for the online testing. Currently, HSC 111-Medical Terminology and AGR 130-Introduction to Animal Science are the only on-line exams. All other Articulation Exams must be obtained from the Perkins IV Office.
