Recommended Documentation for Disabilities
Recommended Documentation for Disabilities
Documentation is needed to verify the disability before any accommodations are made.
Psychological Based Disability
To document any psychological disability, one needs an evaluation that includes a complete diagnosis, including any appropriate test scores and findings, and how the disability impacts the student in an academic setting. The report should include specific recommendations for accommodations as well as an explanation as to why each accommodation is recommended. If accommodations are not clearly identified in the report, DSS will seek clarification or more information. A disability must limit the student's ability to function on an equal basis in the classroom or related activities in order to receive accommodations.
