Recommended Documentation for Disabilities
Documentation is needed to verify the disability before any accommodations are made.
Psychologically Based Disabilities
To document any psychological disability, one needs an evaluation that includes a complete diagnosis, including any appropriate test scores and findings, and how the disability impacts the student in an academic setting. The report should include specific recommendations for accommodations as well as an explanation as to why each accommodation is recommended. If accommodations are not clearly identified in the report, Disabled Student Services will seek clarification or more information. A disability must limit the student's ability to function on an equal basis in the classroom or related activities in order to receive accommodations.
Learning Disabilities
- Psycho-educational evaluation
- Intelligence and achievement tests
Other helpful information would be:
- Learning styles, academic history
- Medications for treatment
- Recommendations from medical certification
- Explanation of physical limitations
- Assessment plan
- Vocational Assessment evaluation
- MUST be current ( within 3 years)
Attention Deficit Disorder (ADD/ADHD/ADS)
Information needed:
- Comprehensive evaluation
- Clinical interview
- Historical data
- Psychological tests
- Medications currently being used
- Clinical observations/recommendations
Physical Disabilities
- A physician's report stating diagnosis
- Medical history with an assessment plan
- Explanation of physical limits
- Medications currently being used
