Madison Haywood Developmental Services - Personal Care and LPN's

Duties

The LPN is responsible for direct support of the Service Recipient in the home and during community activities.  The goal of the LPN is to ensure that all assigned Service Recipients receive the necessary care to achieve a safe, meaningful, and positive home experience.  It is the responsibility of the LPN to develop and teach Service Recipients skills that will enhance their level of independence in the home and community setting.  The goal for all Service Recipients is to achieve total independence in their home and community environments. Ensure all ordered medications and treatments are provided to the Service Recipient according to the Plan of Care and document actions in the Nursing Progress Notes.

Qualifications

LPN's and other personal care staff must be physically able to perform the essential functions of the job as described in the essential duties and responsibilities including: walking, bending, stooping, kneeling, squatting, sitting, reaching overhead and lifting up to fifty (50) pounds.

Must be able to safely lift, transport, and transfer individuals which may include but is not limited to lifting and transferring in and/or out of a vehicle, bed or wheelchair.   As necessary, lifting may be required to assist with the individual’s daily living skills.

QUALIFICATIONS

 Must be 21 years old or older

 Must have High School Diploma/GED

 Must have the ability to deal respectfully with others, to use good judgment and to understand and follow written and oral instructions.

 Must express ideas clearly, both verbally and in writing.

 Special consideration will be given to an applicant with previous experience working with individuals with intellectual and developmental disabilities.

 Maintain valid driver’s license, meet all insurance requirements and be approved by management to be a driver of agency or personal vehicles used during your assigned work hours and have a clear driving record.

 Must successfully complete all required training which includes basic computer and emailing skills.

 Background must be clear of felonies, misdemeanors, domestic violence and/or assault and abuse offenses.

 Must be able to pass a pre-employment drug screening. *Note: We do random drug tests.

Represent MHDS in a professional manner in dealing with the person supported, family members, guardians, other agencies, and other outside professionals as well as in the community.

Perform other duties as assigned.

 

Hours: 8 a.m. to 8 p.m.; 8 p.m. to 8 a.m.
Salary: LPN's $19.85/hour
